Getting Started with Cleveland HeartLab

We offer a reliable and affordable group of biomarkers that offer additive and complimentary insight into a patient’s risk for heart disease and cardiac events.
Getting started with Cleveland HeartLab
- Order a sample collection kit which includes everything needed to draw, store and ship samples back to Cleveland HeartLab.
- Completely fill out the Cleveland HeartLab requisition form. A completed requisition form requires that you:
a. Complete the patient and physician information
b. Check an appropriate ICD-9 diagnosis code
c. Check the test(s) you would like performed
d. Provide the patient’s insurance information (if applicable; see Standard Test Requisition Form below)
e. Provide a physician’s signature
Standard Test Requisition Form: This requisition form is for patients who provide insurance or are covered by Medicare (i.e. insurance/Medicare is billed for services provided, and customer is responsible for Easy Pay amount in addition to a co-pay if applicable).
Client Only Test Requisition Form: This requisition form is for physician offices that would like to be billed directly (i.e. the physician is billed for services provided).
IMPORTANT: If a patient does not have insurance, they will be billed our cash-pay price for services provided. Please contact Cleveland HeartLab for our cash pricing.
- Ship samples to Cleveland Heartlab.
