Use: Cholesterol is a steroid with a secondary hydroxyl group in the C3 position. It is synthesized in many types of tissue, but particularly in the liver and intestinal wall. Approximately three quarters of cholesterol is newly synthesized and a quarter originates from dietary intake. Cholesterol assays are used for screening for atherosclerotic risk and in the diagnosis and treatment of disorders involving elevated cholesterol levels as well as lipid and lipoprotein metabolic disorders.
Methodology: Enzymatic Photometric
Goal Value: <200 mg/dL
Additional Information: Total cholesterol can be used to evaluate hypo- or hyperlipidemia
Calculation to determine LDL-C using a standard lipid panel:
Total cholesterol-Direct HDL cholesterol-(Triglycerides/5)
Goal Value: <100 mg/dL
CPT Code: 83718
Order Code: C118
Synonyms: Alpha- Lipoprotein Cholesterol, HDL-C
Sample Type: Serum
Minimum Volume: 1.0 ml
Tube Type: Tiger top tube
Patient Preparation: Fasting or non-fasting sample may be used.
Collection:
1. Draw sample and gently invert 5 times (DO NOT SHAKE)
2. Let the blood clot for 30 minutes
3. Centrifuge sample at 1300 rcf for 30 minutes
Storage Instructions: Cleveland HeartLab recommends that the sample be shipped same day as draw. The sample is stable at 6 days 4°C.
Causes for Rejection: Samples will be rejected if the sample is improperly labeled
Use: High density lipoproteins (HDL) are responsible for the reverse transport of cholesterol from the peripheral cells to the liver. Here, cholesterol is transformed to bile acids which are excreted into the intestine via the biliary tract. Monitoring of HDL-cholesterol in serum is of clinical importance since an inverse correlation exists between serum HDL-cholesterol concentrations and the risk of atherosclerotic disease. Elevated HDL-cholesterol concentrations are protective against coronary heart disease, while reduced HDL-cholesterol concentrations, particularly in conjunction with elevated triglycerides, increase the cardiovascular risk.
Methodology: Enzymatic Photometric
Goal Value:
Male: ≥40 mg/dL
Female: ≥50 mg/dL
Note: If a gender is not provided, the result will default to the male goal value.
Additional Information: HDL cholesterol levels are temporarily low during acute illness, immediately following a heart attack, or during stress (like from surgery or an accident). Following illness, HDL cholesterol levels should be measured after at least six weeks. HDL cholesterol levels may also change during pregnancy, and therefore levels should be measured after at least six weeks following birth.
